All financial assistance should be applied for by the family's social worker through calling 1-888-356-5253 or emailing staci@jakesfoundation.org. Sufficient documentation of need will be required. Specific guidelines will be used to determine qualifying families. Limitations and restrictions will also be used to ensure that assistance is not abused.
